How do I submit a child/youth safety related concern or report?

Child/youth safety related concerns and reports can be submitted in multiple ways. You can review each method below:

  1. Click on "submit a ticket" and select "Report a Safety Concern or Incident" in the department dropdown to directly access the reporting form
  2. Email: safeguarding@uscjusy.zohodesk.com
  3. Scan the QR code on a Child/Youth Protection poster to fill out the reporting form
